Case Management

REACH for Success

SWADC offers Case Management services to low/moderate income individuals and families.  Case Management includes goal setting, and advocacy on behalf of the client.  The goal of this program is to bring families out of poverty and into a self-sufficient lifestyle.  Some activities include educational pursuits, job referrals, budget counseling and other helpful tools.  The only eligibility requirement for Case Management services is that a household fall below 200% of the Federal Poverty Level.
